Consent to participate in MDbio's Health Improvement Program

This document outlines important information about MDbio's Health Improvement Program, including its purpose, procedures, risks, and possible benefits. 

Program Purpose and Procedures

• The Health Improvement Program aims to gather research data for product development.
• By consenting, you agree to participate in all trial procedures. Withholding consent may result in forfeiture of participation.
• Participation may or may not provide therapeutic benefits to you. The information gathered will contribute to product research.

Health and Safety

• Although the trial product is unlikely to cause adverse effects, you agree to immediately consult MDbio medical professionals if you experience any mild medical conditions. Our team will assist with dosing adjustments and side effect management.
• We do not recommend individuals who are currently pregnant or undergoing fertility procedures to participate in MDbio's Health Improvement Program.

Product Ingredients and Legal Considerations

• MDbio +hemp product lines are manufactured with full-spectrum hemp extracts and botanical ingredients, which may include trace, non-psychoactive quantities of THC, adhering to the legal limit of 0.3% THC under the 2018 Hemp Farm Bill.
• MDbio is not liable for any drug test results you may encounter due to product usage.

Data Usage and Privacy

• Your anonymous data may be shared with internal and external third parties that MDbio deems appropriate for research purposes. Personal data, including your name, address, phone, and email, will NOT be shared outside MDbio.
• You may request the removal of your anonymous data after the study is complete. However, requesting removal during the trial may result in withdrawal from further participation.

Conduct and Compliance

• You agree not to participate in any other research studies during this trial without MDbio approval, to prevent adverse effects from overlapping nutraceutical treatments.

• If you withdraw from the program for personal reasons or fail to comply with instructions, your data may not be included in the research findings. Additionally, you forfeit claims to financial compensation, such as discounted products or other in-kind benefits.

• Trial managers may remove you from the program if it is in your best interest or due to non-compliance or other critical reasons.
